Hi,

Have a question...A patient on long-term warfarin for more than 5 years for PE/DVT, gets admitted for some new problem. If you find the INR to be subtherapeutic, would you consider heparin bridging? Do we have to bridge every time we find the INR to be low? How would coumadin clinics/anticoagulation clinics take care of the situation?
